The Medical Record & Registration Department plays a critical role in maintaining the integrity, accessibility, and confidentiality of patient data. By ensuring meticulous record-keeping and seamless documentation, this department supports clinical services, administrative processes, and patient welfare schemes.

From the moment a patient steps in for diagnosis, through treatment and follow-ups, until discharge or reimbursement — this team ensures every medical event is documented, traceable, and aligned with institutional and legal standards.

 

Key Functions

Patient Registration & Case Documentation

  • New Case and Special Investigation Card Registration for streamlined diagnosis and treatment
  • Management of Old Case Files, including timely issue and deposit for follow-ups or reference.

Patient Welfare & Concession Support

  • Verification and sanctioning of applications under various government and institutional patient welfare schemes for free or concessional cancer treatment.

Hospital Death Registration

  • Accurate and timely documentation of hospital death records as per legal and public health requirements.

Travel Concessions for Patients

  • Issue of ST/Railway concession passes to eligible cancer patients for medical travel support.

Additional Support Services

Patient Assistance

  • Assistance with Death Claims/Mediclaim form fill-ups.
  • Preparation and issue of Medical Reimbursement documentation.
  • Issuance of computerised Medical Certificates for leave, treatment, and compliance.

Digital Communication

  • Phone call and SMS alerts for scheduled OPD appointments, reducing patient wait times and ensuring attendance.

Statistical Recordkeeping

  • Maintenance of Hospital General Statistics, including patient demographics, admission rates, treatment patterns, and mortality data — useful for research, policy, and quality control.
